29F, G2P1, 32 weeks by dates. Feeling well. Good fetal movements (>10/day). No PV bleeding, no contractions, no visual disturbance or epigastric pain.
O
Objective
BP 118/72. SFH 31 cm (normal for gestation). FHR 142, regular. No oedema. Urine: no protein, no glucose.
A
Assessment
32/40 uncomplicated pregnancy. Iron-deficiency anaemia — improving (Hb 10.8 → 11.4).
P
Plan
Continue folic acid + iron. Growth scan at 36/40. Tdap done. Review in 2 weeks. Patient education on pre-eclampsia warning signs.
